Will you train a private model for us?

Only if the product needs it. Many systems work with a strong hosted model plus your documents and tools. I will say when fine tuning is worth the cost.

Can the agent work on our existing phone system?

Often yes. I have shipped agents on Twilio media streams and on Asterisk into the phone network. The first step is how your calls arrive today.

Is this safe for clinics or regulated data?

The healthcare systems I work on are built around access control, your existing practice software, and the compliance pack you already follow. I will not promise a certificate you have not scoped.
